Tashkent, Uzbekistan (UzDaily.com) -- An algorithm for analyzing the condition of a patient with Covid-19 in the first 4 days and a forecast of how the disease will behave further, drugs that have shown their effectiveness at different stages of the disease and other pressing issues were discussed during an international webinar held on 22 December 2020 by the National Chamber of Innovative Healthcare of the Republic of Uzbekistan.
Specialists from Great Britain and India presented their presentations at the webinar. About 100 participants in the practical discussion included representatives of the Legislative Chamber of the Parliament of the country, specialists from medical organizations of the Ministry of Health of Uzbekistan, including the Research Institute of Epidemiology of Microbiology and Infectious Diseases, regional health departments, regional infectious diseases hospitals, RSNPMTSOiR, RSCEMP, Tashkent Medical Academy, Center for the Development of Professional qualifications of medical workers, Tashkent Pharmaceutical Institute.
An overview of the latest treatment protocols for Covid-19 was reviewed in his presentation by Dr. Ajay Gupta, Director of the Foundation Program at Royal London Hospital and co-editor of the American Heart Association magazine. He noted that according to international data, the United States and India are the first in the number of patients diagnosed with Covid-19 - 16.3 million and 9.9 million cases, respectively. At the same time, these countries also have a high mortality rate - 1.83% (USA) and 1.46% (India). In total, about 72 million cases of Covid-19 have been registered in the world today. Uzbekistan copes well with the epidemic - there are about 75,240 cases in the country, and the case fatality rate is 0.86%.
Dr. A. Gupta presented the latest data based on the exchange of international experience on the main symptoms of the disease, the extrapulmonary manifestations of Covid-19, the characteristic features of various forms of the course of the disease, the patient management algorithms adopted in different countries and, accordingly, the use of drugs. Experts all over the world consider it necessary to avoid polypharmacy, that is, the use of a large number of drugs, which can worsen the healing process.
In particular, it was noted that for non-hospitalized mild and moderate forms of the course of Covid-19, the use of dexametozone is not recommended. Also, it is not recommended for hospitalized, but not requiring oxygen support, the form of the disease. In the latter case, there is also insufficient data to recommend or oppose the routine use of remdesivir. However, it can be useful for patients at high risk of disease progression.
Thus, each form of the course of the disease has its own indications, and each drug should be applied based on the analysis of the condition of a particular sick person.
The great interest in this topic and theses of the presentation was evidenced by many questions from participants from all regions of Uzbekistan.
During the webinar, a dialogue between foreign experts and the Uzbek side took place, which led to specific agreements. In particular, Dr. Ajay Gupta and a member of the Advisory Council of the National Chamber, Doctor of Medical Sciences, Professor Asamidin Kamilov decided to work together to develop an algorithm for predicting the development of Covid-19 in the first 4-5 days of illness for practitioners. Many countries have experience in this area. It is planned to analyze the algorithms developed by specialists from the USA, Great Britain, as well as the experience of specialists from Uzbekistan and, on their basis, offer doctors the most optimal and practical forecasting option.
The insidiousness of Covid-19 lies in the fact that in the first days the sick person can feel quite well, and on the 5-6th day they feel a sharp deterioration in their condition. The algorithms will allow practical doctors in the first days of the disease to predict the possibility of such a deterioration and prevent the development of an unfavorable course of Covid-19.
During the webinar, the founder and chief radiologist of Mahajan Imaging (New Delhi, India), Dr. Harsh Mahajan, introduced the participants to the latest international experience in radiological diagnostics of Covid-19.
Along with the latest transcripts of computed tomography of the lungs (MSCT), participants were interested in the experience of using chest x-ray to diagnose Covid-19. While this method is considered less sensitive than MSCT, it is more readily available, especially in regions where x-rays are always available. Thus, having certain skills in analyzing chest X-ray, a doctor can more accurately diagnose and treat a patient with Covid-19 almost anywhere.
The National Chamber also studied in detail the experience of Mahajan Imaging in the use of artificial intelligence in the field of radiology, as the most promising for joint implementation in the healthcare system of Uzbekistan.
The webinar allowed practitioners to familiarize themselves with the latest international experience and recommendations for diagnosing the condition of patients with Covid-19 at different stages of the disease and the associated individual treatment. A partnership was discussed, which could be another step towards effectively combating the spread of coronavirus infection.
