Uzbekistan proposes to simplify the form of a questionnaire for issuing a passport for traveling abroad

Tashkent, Uzbekistan (UzDaily.com) -- The Ministry of Internal Affairs of Uzbekistan proposes to simplify the form of a questionnaire for issuing a passport of the citizens to travel abroad.

The draft order on approval of the form of a questionnaire for issuing a passport of the citizens to travel abroad has been published for discussion.

The form consists of 11 items. According to the document, the questionnaire is filled in by an employee of the data collection point, and the applicant signs it.

The questionnaire will include the last name, first name and patronymic, gender, date and place of birth, telephone number and email address, information about the passport of a citizen of Uzbekistan, information about access to information constituting state secrets, information about places of work (studies and military service ) for the last five years and data about the spouse.

The draft order also proposes to approve a separate form of application for registration of a passport for children under 16 years of age.
